When purchasing hole in one insurance, US Hole In One makes the process simple and easy. Once a customer receives a quote either online or over the phone and via email, all that is left to do is to fill out the application to get the coverage in place for their upcoming tournament. How will they get the application for the hole in one coverage? It will be emailed over to the customer along with the quote they receive. US Hole In One’s application is broken down in to 7 sections. Here is a breakdown of the 7 sections and what is needed to complete the application.

• Section 1 :: CONTACT INFORMATION
  • This section includes your company or organization name, contact name, mailing address, daytime phone number and email address. The information in this section should for the most part be completed for you already thanks to the tournament consultants or the information you entered on the online quote request. If the shipping address for the signage is different than your contact information, make sure to fill that in under the shipping address which is also in section one.

• Section 2 :: TOURNAMENT & GOLF COURSE INFORMATION
  • The name of the golf tournament along with the golf course and the city and state it is located in is indicated in this section. If there are multiple courses at the course, the names of the course(s) that is being played must be indicated on the application in this section next to the name of the course. Be sure to remember US Hole In One can not provide hole in one insurance unless the coverage is part of a golf tournament.

• Section 3 :: GRAND PRIZE HOLE(S) INFORMATION
  • Only the grand prize hole(s) information is completed in this section. Complete one line per each grand prize hole. On each line you will complete the contest date, which is the date of the tournament, number of golfers, amateurs and professionals, hole number, yardage, prize value, prize description and the certificate fee. This information will all be on the quote that your tournament consultant sent over or the quote sent over after you requested one online so you can easily transfer it over. If you are playing multiple courses during the tournament, next to the hole number it must be indicated what course the grand prize hole is on. This section will also ask you if you need a sign for your grand prize hole, along with the sponsor name for that sign.

• Section 4 :: BONUS PRIZE HOLE(S) INFORMATION
  • This section asks if you want bonus prizes and how many, for the other par three’s on the course that are 130 yards or longer. Included for free in your package is the standard bonus prizes or you can upgrade to the premium for a $45 fee. We also include standard signs for those holes at no additional charge or you can upgrade to custom signs for a $30 fee with a sponsor name on them.

• Section 5 :: ADD ON CONTEST COVERAGE
  • When purchasing hole in one insurance you also have the option to add on other contests at a discounted rate. If you would like to also purchase coverage for an add on putting contest or dice roll contest, you would indicate that  in this section. In addition to that, you would also complete the application for the add on contests.

• Section 6 :: SHIPPING & PAYMENT INFORMATION
  • Hole in one insurance can be purchased via check or credit card. In this section you indicate payment information by entering your credit card information or choosing to be invoiced.

• Section 7 :: IMPORTANT
  • In the final section, US Hole In One asks that you sign off that you agree with the terms and conditions.

Once completed, the hole in one application can be faxed to 610-525-8004 or emailed to orders@usholeinone.com). After it is received by an US Hole In One tournament consultant, your application will be processed and your coverage will be put in place for you golf event.
