US Hole In One tries to make the process of getting hole in one insurance simple and painless. After clients get a quote either online or over the phone, the next and final step is to fill out the application. The application will be emailed over to the customer along with the quote and the client will be instructed to download and print before they can fill it out. The hole in one application is broken down to seven sections. Here are instructions so you do not miss anything while completing the application.

Section 1 – This section asks you for your contact information as well as your company name. The information in this section should for the most part be completed for you already thanks to the tournament consultants. If the shipping address for the signage is different that your contact, make sure to fill that in under shipping address also in section one.

Section 2 – This is where the golf course information as well as the golf tournament name is indicated. Be sure to remember US Hole In One can not provide hole in one insurance unless the coverage is part of a golf tournament.

Section 3 – This is the most important section on the application. Section 3 asks you to fill in all the hole in one coverage information like the date, number of golfers, hole number, yardage, prize value, prize description, and the certificate fee. This information should all be on the quote that your tournament consultant sent over so you can easily transfer it over. This section will also ask you if you need a sign for your grand prize hole, along with the sponsor name for that sign.

Section 4 – This section asks if you want bonus prizes and how many, for the other par three’s on the course that are 130 yards or longer. Included for free in your package is the standard bonus prizes or you can upgrade to the premium for a $45 fee. We also include standard signs for those holes at no additional charge or you can upgrade to custom signs with a sponsor name on them.

Section 5 – If you would like to also purchase coverage for an add on putting contest or dice roll contest, you would indicate that  in this section. In addition to that, you would also complete the application for the add on contests.

Section 6 – Check or credit card is accepted as payment for your hole in one insurance. In this section you indicate payment information by entering your credit card information or choosing to be invoiced.

Section 7 – In the final section, US Hole In One asks that you sign off that you agree with the terms and conditions.
